Our Home Health Licensed Clinical Social Workers enjoy some excellent benefits: * Earn $85 - $95 per visit * Competitive, weekly pay with travel time and mileage reimbursement * Holiday and PTO pay for full-time employees * Supportive, caring management that will have your back * Medical for full-time, dental, vision, and supplemental benefits available for everyone * 1:1 patient ratio; where you make a difference in our patients’ care with flexible assignments to fit your needs * Continuing education benefits and discounted courses through Colorado Christian University and Rasmussen University As a Home Health Licensed Clinical Social Worker, here’s a big-picture view of what you’ll do: * Provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or lack of resources that make managing their injury or disease difficult * Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT, OT, or SLP, focused on the patient’s plan of care and individual goals * Monitor a patient’s psychosocial condition and identify social and emotional needs * Conduct patient assessments, document progress, and ensure patients are moving toward goals * Provide counseling, community resource planning, crisis intervention, and advocacy * Consult with family and caregivers on the patient’s plan of care and how to support progress A few must-haves for Home Health Licensed Clinical Social Workers: * Master’s degree in Social Work and active LCSW license in Arizona * Minimum of one (1) year of social work experience, ideally in home healthcare * CPR Certification (demonstration course required) * Valid Arizona Fingerprint Card * Knowledge of state and federal home health regulations * Strong interpersonal skills; empathetic, compassionate, resourceful, and an effective communicator Why Work for Interim HealthCare?
